Requests are processed at the end of each business day and will be ready the morning of the next business day. Questions? Contact account@uoregon.edu.

Directions: Fill out Section 1 or Section 2 as needed. Then fill out Section 3, including a copy of your photo ID. Print and fax form to (541) 346-1722.

Section 1: Duck ID

To have your Duck ID password changed, enter your Duck ID username and a new, temporary password below:

This temporary password will give you access to pick a new password. To select a new password, visit duckid.uoregon.edu. You will not be able to access other services using your Duck ID until you have set a new password.

Password requirements for Duck ID:
  • between eight and 32 characters in length
  • at least one number and at least one letter
  • not one of your last three passwords
  • not your email address or name
  • not a word in the English or Spanish dictionaries
  • not a common keyboard sequence (asdfghjk or qwertyui, for example)

Section 2: BANNER and AppWorx

To have your BANNER or AppWorx password changed, enter your BANNER username and new password below.

BANNER passwords must begin with a letter, contain at least 1 number, and be a minimum of 8 characters.
